all necessary forms.
Forms
-Make sure that all the forms are completed and turned
in to the office by the first day of practice (These include
the physical, parent consent form and co-curricular agreement).
The $95 sports participation fee must also be paid to Mrs. Spray.
Please let the coaches know if you can not pay the fee. (click
here for forms)
(Physicals are good for two years. We recommend that you get a physical
your Freshman and Junior years)

You will need to print a copy to have during the first week of practice.
